MRI Triage Guidelines (last updated 6/21/2026)Wesley Medical CenterPurpose: To provide guidance on completing MRI exams in a timely manner by following standard guidelines for triaging Trauma, STAT and Routine orders. Protocol: 1. Trauma orders take precedent and will not be placed behind any other order. 2. STAT exams are done following date and time ordered and must meet appropriate diagnosis for STAT orders listed below. a. Acute stroke with altered mental status (AMS)b. Acute basilar occlusionc. Acute hydrocephalusd. Acute cord compression/Cauda Equina syndromee. Appy – appendix on pregnant women if ultrasound exam is indeterminatef. Cerebral herniationg. Carotid or vertebral artery dissectionh. Meningitis/cerebritisi. MRV – Cerebral venous thrombosisj. MCRP – Abdomen wo contrast for obstructive bile duct or dilation seen on ultrasoundk. Ovarian torsion if ultrasound exam is indeterminatel. Post TPA/TNK for stroke thrombosis: scan within 24 hours after medication is administered.m. Post intracranial thrombectomy of a large vessel occlusion or concern for hemorrhagen. Traumatic spine with neuro symptoms or diagnosed on CT for ligament injuryo. Traumatic brain injury or Anoxic3. STAT exams ordered without an approved diagnosis, will be considered a routine order and follow routine order placement. 4. Routine exams, regardless of when ordered, including outpatient exams, do not supersede a true STAT order. a. Routine orders with pending discharge shall not take precedent over other routine orders. Routines are completed in order of date and time ordered, patient clearance for MRI exam and patient readiness. 5. Requests for escalation of a study go to the resident on duty. Approved escalations must have reasonable concern for patient wellbeing. If further escalation is required, the radiology attending physician on duty or Dr. Matchette may be called for further discussion. a. Approved escalations are placed at the end of the STAT orders. · Andover will transfer patients 5p-9p
· 7a –7p -- If call is uncovered, Hillside will cover

· Testicular/Ovarian Torsion
· Liver/GB
· 1st trimester for ectopic with positive pregnancy test.
o Ultrasound will not come in for previously documented IUP
or symptoms of nausea and vomiting.
o Pregnancy detailed anatomy scans will preferably be done 7a- 5p
· DVT – with positive D-Dimer
· Pediatric exams for appendicitis and intussusception
